Can excessive lower-eyelid fat removal cause a hollow appearance?

Yes. Removing too much fat can leave the lower eyelid looking hollow. Planning needs to consider fat bulges, existing hollows and eyelid support together.

THE CLAIM BEING CHECKED

„Excessive fat removal during lower-eyelid surgery can accentuate a hollow appearance.“

True statement

THE ANSWER

What is true about it?

Yes, excessive removal of lower-eyelid fat can cause or accentuate a hollow or skeletonized appearance. The scientific overview of lower-eyelid surgery describes this relationship and the shift towards conservative removal or fat repositioning. That does not mean every instance of fat removal is wrong, or that repositioning is automatically the better choice for every person.

A CLOSER LOOK

The context behind the claim.

Under-eye bags and hollows at the eyelid–cheek junction can coexist, even though they appear to be opposite problems. The American Society of Plastic Surgeons therefore describes both removal and repositioning as possible surgical steps. The appropriate combination depends on starting anatomy and tissue support; a single before-and-after post establishes neither the superiority of a technique nor that of a particular surgeon.

YOUR NEXT STEP

Making a more informed decision.

Before surgery, ask which tissues would be removed, preserved or repositioned, and how existing hollows would be addressed. Check My Treatment offers an individual written or video assessment by independent specialists in plastic and aesthetic surgery, using structured review models and transparent criteria. Its offering page sets out the review models and availability; individual assessments were most recently listed with waiting-list availability.
